Risk factors for multidrug-resistant bacterial infection among patients with tuberculosis

結核治療薬それ自体の抗菌活性、および広域スペクトル抗菌薬が広範に使用されていることを考慮すると、結核治療を受ける患者に多剤耐性菌が出現する可能性は高い。症例対照研究により、結核患者における多剤耐性菌感染症の臨床的予測因子を調べた。1996 年 1 月 1 日から 2006 年 8 月 31 日に、結核と診断され治療を受けた患者から症例および対照を選択した。多剤耐性菌感染症が認められる結核患者を症例、非多剤耐性菌感染症が認められる結核患者を対照とした。多剤耐性菌感染症のリスク因子を明らかにするために多重ロジスティック回帰分析を行った。研究期間中に 3,667 例が結核と診断され、その治療を受けた。123 例に細菌感染症エピソードが認められ、このうち 59 例（48.0％）は 1 回以上、多剤耐性菌に感染していた。慢性腎不全（補正オッズ比［OR］4.96、95％信頼区間［CI］1.37 ～ 18.01）、および 3 か月以内の標準的結核治療薬以外の抗菌薬使用（補正 OR 4.37、95％CI 1.74 ～ 10.95）が、多剤耐性菌感染症の独立リスク因子であった。結核患者の細菌感染症の多くは多剤耐性であった。医師は、慢性腎不全を有する結核患者や他の抗菌薬を最近使用した結核患者における多剤耐性菌感染症の可能性を認識しておくべきである。
