Candida species bloodstream infection: epidemiology and outcome in a single institution from 1991 to 2008

カンジダ血症は、依然として医療施設内での疾患・死亡の主要な原因である。本研究の対象は、1991 年から 2008 年に単一施設で実施された血液培養サーベイランスプログラムにおいて、前向きに記録されたカンジダ（Candida）属菌血流感染症エピソードとした。菌種や診断後 30 日時点の生存率などのカンジダ血症エピソードに関するデータを解析した。研究期間中に 529 件のカンジダ血症エピソードが認められた（このうち院内感染症は 495 件）。カンジダ・アルビカンス（C. albicans）以外の Candida 属菌によるカンジダ血症の発生率（52％）は、C. albicans（48％）よりも高かった。全体の 30 日粗死亡率は 32％であった。カンジダ・パラプシローシス（C. parapsilosis）によるカンジダ血症患者の死亡率が最も低かった（23％）。カンジダ・クルーセイ（C. krusei）によるカンジダ血症との関連が高かった因子は、血液悪性腫瘍（61％、P ＜ 0.001）、幹細胞移植（22％、P ＝ 0.004）、好中球減少症（57％、P ＝ 0.001）、およびアゾール系抗真菌薬使用歴（26％、P ＜ 0.001）であった。本研究で 30 日粗死亡率が最も高かったのは C. krusei によるカンジダ血症患者であった（39％）。カンジダ血症の臨床的・微生物学的特性を明らかにするため、および各種状況下での経験的治療法の指針を得るにためには疫学研究が重要である。
