クロストリジウム・ディフィシル(Clostridium difficile)感染症患者の死亡率については、標準化されたデータがほとんどない。文献の多くは、C. difficile 感染に「関連する(attributable)」死亡率を報告しているか、普遍性がない死亡の指標を採用している。本研究の目的は、C. difficile 感染症患者の大規模な非選択的コホートを対象として、全原因死亡率のパターンと傾向を調査することである。総合病院 3 施設と地域病院 7 施設からなる国民保健サービス(NHS)トラストで、2002 年から 2008 年に糞便の毒素検査陽性により C. difficile 感染症と診断された全患者を対象に、後向きコホート研究を実施した。2 つの情報源に基づいて患者の生存状況を判定した。合計 2,571 例の初回 C. difficile 感染症患者を特定した(女性 1,638 例、年齢中央値 82.1 歳)。累積死亡率は 7 日目 13.4%、14 日目 20.8%、30 日目 32.5%、および 1 年目 58.7% であった。死亡率には性別、診断年、または病院による有意差は認められなかった。30 日死亡率は、40 歳未満の 3.4% から 90 歳超の 41% に段階的に増加した。死亡率はこれまでの研究の報告よりも有意に高かったが、調査期間全体にわたり、またトラスト内の病院間で高度に一致していた。予後は年齢の上昇とともに悪化したが、30 日絶対死亡率が高かったことは、本研究コホートの年齢から説明できると考えられる。C. difficile 感染症は早期の死亡率が高い。死亡率を低下させるためには、新規の介入を診断後速やかに開始する必要がある。C. difficile 感染症の転帰に関する標準化されたデータが求められる。
