Evaluation of the bactericidal efficacy of three different alcohol hand rubs against clinical isolates of Staphylococcus aureus using an ex vivo carrier test

黄色ブドウ球菌（Staphylococcus aureus）に対する 3 種類の擦式アルコール製剤の効果を ex vivo の担体試験（carrier test）により評価するとともに、擦式アルコール製剤の残存効果、および機械的な摩擦の効果を調べた。Ex vivo 検査で殺菌効果を得るために要する擦式アルコール製剤との接触時間（10 分から 20 分以上）は、in vitro 検査よりもはるかに長かった。機械的な摩擦を併用した擦式アルコール製剤の効果は、機械的摩擦のみの対照群よりも高かった。擦式アルコール製剤には残存効果は認められなかったため、蒸発時間（15 秒）よりも長い接触時間を採用した今回の ex vivo 検査でみられた殺菌効果を、実地臨床で得ることは困難であると考えられる。これらの結果を考慮すると、実地臨床において擦式アルコール製剤によって顕著な殺菌効果（4 log₁₀ 以上の減少）が得られる可能性は低く、医療従事者の手指汚染減少の程度は交差汚染や医療関連感染のリスクを払拭するほどではないことが示唆される。擦式アルコール製剤には残存効果が認められなかったため、医療従事者の手指に保菌が再度生じることを予防することはできないと考えられる。

ごもっともな考察であるが、クロルヘキシジン等の合剤による同様の比較試験も実施すると、これらの効果が検証できて良いだろう。
