Cost and outcomes of nosocomial bloodstream infections complicating major traumatic injury

本研究の目的は、外傷関連の院内血流感染症（BSI）の発生率、転帰、およびコストの評価である。本研究は行政区域で集約化された成人外傷センターまたは小児外傷センターに、4年間の研究期間中に重度の外傷（外傷重症度スコア［ISS］12以上）のため入院した患者を対象としたマッチドコホート研究であり、事前に規定した基準に基づいて、院内BSI症例とBSIが認められない対照を1：3でマッチさせた。死亡率、入院期間、および院内BSIに起因するコストなどの転帰を評価した。57例のBSI症例を特定し、このうち3例ずつの対照とマッチさせることができたのは51例であった。BSI症例のISS平均値は30.3であり、最も分離頻度が高い病原体は黄色ブドウ球菌（Staphylococcus aureus）であった（27％）。BSI症例群の入院期間は相対的に27％延長した（P ＝ 0.02）。BSI症例群の30日死亡率のオッズ比は5.8（95％信頼区間［CI］1.1～30.8、P ＝ 0.04）であった。生存者を対象とした群間比較では、BSI症例群の総医療費の相乗平均値は相対的に53％増加した（BSI症例群97,993ドル［95％CI 70,143～136,899ドル］、対照群62,297ドル［95％CI 52,155～74,411ドル］、P ＜ 0.0001）。本研究から、重度の外傷に合併した院内BSIでは入院期間および医療費が大幅に増加することが初めて示された。このデータは、BSIによる外傷受傷者への有害な影響を軽減するための取り組みが必要であることを示すものである。
