Costs of nosocomial pneumonia caused by meticillin-resistant Staphylococcus aureus

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）による院内感染症は医療費および経済的損失の増大をもたらす。本研究の目的は、MRSA院内感染肺炎に要するコストをメチシリン感性黄色ブドウ球菌（meticillin-susceptible S. aureus；MSSA）肺炎と比較することである。2005年1月から2007年12月に入院したMRSAまたはMSSAのいずれかによる院内感染肺炎患者を対象として、症例対照研究を行った。下記の特性に基づいて、症例（MRSA肺炎）に対照（MSSA肺炎）をマッチさせた。年齢、基礎疾患の重症度、集中治療室および非集中治療室への入室期間、同一年での入退院、および肺炎の診断前の入院期間がMRSA肺炎症例よりも長いこと。82例（症例41例、対照41例）の解析を行った。院内感染MRSA肺炎患者の総コストはMSSA肺炎患者よりも有意に高かった（60,684ユーロ対38,731ユーロ、P ＝ 0.01）。MRSA肺炎に起因するコストは患者1例あたり17,282ユーロであった（P ＜ 0.001）。経済的損失はMRSA肺炎患者のほうがMSSA肺炎患者よりも大きかった（11,704ユーロ対2,662ユーロ、P ＝ 0.002）。入院期間中の死亡例は症例群のほうが対照群よりも多かった（13例対1例、P ＜ 0.001）。病院職員はMRSA肺炎に起因するコストを認識し、MRSA伝播を予防するための制御対策を導入すべきである。
