Surveillance Programme for Healthcare Associated Infections in the State of Sao Paulo, Brazil. Implementation and the first three years’ results

医療関連感染に関するデータの収集・分析には、政府がそのためのプログラムを開発する必要がある。本稿ではブラジルの医療環境について、およびサンパウロ州医療関連感染サーベイランスプログラム（Programme for Surveillance of Healthcare Associated Infections in the State of Sao Paulo；PSHAISP）の2004年から2006年におけるプログラムの実施とその中間結果について述べる。医療環境の特性の評価は全国的データベースを用いて実施した。PSHAISPは急性期病院または長期ケア施設の各部門を対象として実施した。急性期病院のサーベイランス対象部門は、外科部門、集中治療室、および高リスク新生児室とした。サーベイランス対象の感染症は、清潔手術での手術部位感染症、肺炎、尿路感染症、デバイス関連血流感染症とした。長期ケア施設部門については全入院患者を対象として、肺炎、疥癬、胃腸炎の報告を行った。プログラム実施1年目は457の医療施設が参加したが、これは全国的データベースに登録された病院の51.1％に相当する。本研究で得たデータは初期の結果ではあるが、すでに医療関連感染のサーベイランスと予防の両方の教育に活用されている。PSHAISPの結果は、多数の病院からデータ収集が実施可能であることを示しており、サンパウロ州による介入効果の評価、および資源配分にあたって有用であると考えられる。
