Excess costs associated with common healthcare-associated infections in an Iranian cardiac surgical unit

開胸手術後の医療関連感染症は、死亡および罹患の主要な原因となるだけではなく、コストの増加をもたらす。非西洋諸国の医療関連感染症による追加コストに関するデータは限られている。イラン人集団を対象として4種類の一般的な医療関連感染症の直接コストを推計するために、待機的開胸手術のために入院した1,191例の調査を行った。医療関連感染症の定義には全米病院感染サーベイランス（NNIS）の基準（米国疾病対策センター［CDC］、米国、ジョージア州、アトランタ）を使用した。入院中の1日あたりの室料、臨床検査費、薬剤費、医療器具費、および手術室使用料を病院経理部門が提示した。医療関連感染症患者の直接医療費の追加コストに対する医療関連感染症の寄与を多変量線形回帰分析を用いて評価した。対象集団の平均年齢は57.3 ± 11.9歳、男性が857例（72.0％）であり、64例（5.4％）に医療関連感染症が発生した。全体で73件の感染症が認められ、頻度が高いものは手術部位感染症（49.3％）、次いで尿路感染症（20.5％）、血流感染症（16.5％）、および肺炎（13.7％）であった。その他の交絡因子の補正後も、医療関連感染症は直接医療費の追加と関連していた（β ＝ 1,707.06、標準誤差 ＝ 90.84、P ＜ 0.001）。医療関連感染症患者の医療費は医療関連感染症がない患者の約2倍であった。入院期間の延長が追加コストの半分以上に寄与していた。
