Bacteraemia caused by non-freundii, non-koseri Citrobacter species in Taiwan

本研究では、まれなシトロバクター（Citrobacter）属菌による菌血症の臨床的特性を解析した。シトロバクター・フレウンディー（C. freundii）およびシトロバクター・コセリ（C. koseri）以外のすべてのCitrobacter属菌分離株の種の同定を、2種類の市販同定システムと16S rRNA遺伝子配列解析により行った。2000年1月から2009年12月に、Citrobacter属菌による菌血症患者を合計306例特定した。4例（1.3％）がシトロバクター・ブラアキー（C. braakii）による菌血症、1例がシトロバクター・アマロナティカス（C. amalonaticus）による敗血症、1例がシトロバクター・セドラキー（C. sedlakii）による敗血症であった。C. freundiiおよびC. koseri以外のCitrobacter属菌であると誤同定された分離株は、Phoenix自動システムPMIC/ID-30では8株、Vitek IIシステムでは3株であった。C. freundiiおよびC. koseri以外のCitrobacter属菌による菌血症患者6例中5例（83.3％）は医療関連感染症であり、また5例（83.3％）は腹腔内感染からの続発症であった。頻度の高い基礎疾患は癌および肝硬変であった。6例中2例（33.3％）が死亡した。抗菌薬感受性検査から、Citrobacter属菌の種によって耐性パターンが異なることが示された。C. freundiiおよびC. koseri以外のCitrobacter属菌は同定が困難であり、免疫不全患者において二次性医療関連菌血症を引き起こす腹腔内感染症のまれな起因菌である。
