Natural history of meticillin-resistant Staphylococcus aureus colonisation among residents in community long term care facilities in Spain

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の拡散は、急性期ケア病院や長期療養施設入居者の大きな問題となっている。著者らは長期療養施設入居者における MRSA保菌の自然経過をコホート研究で評価した。入居者の2つのコホート（MRSA保菌者231例、非保菌者196例）を18か月間にわたって追跡し、6か月ごとに鼻腔および褥瘡の培養を行った。MRSA保菌者のコホートでは、MRSAの6か月以上の持続的保菌者は110例（47.8％）、一時的保菌者44例（19.0％）、間欠的保菌者9例（3.9％）であった。MRSAの持続的な保菌のリスク因子は特定できなかった。MRSA保菌の1年間における発生率は約20％［95％信頼区間（CI）14.3～25.5］であった。抗菌薬投与はMRSA保菌と独立して関連していた（オッズ比2.27、95％CI 1.05～4.88、P ＝ 0.03）。パルスフィールド・ゲル電気泳動法および複数部位配列タイピング（multilocus sequence typing）により識別可能であったのは、CC5-MRSA IV（スペインの病院に広く蔓延）とST22-MRSA IVの2クローンのみであった。この研究により、地域の長期療養施設におけるMRSAの疫学に関する知見が得られた。これらの知見はスペインの長期療養にとって重要である。

この研究は保菌状態の持続の有無についてのみ着目されており、臨床的な感染症の発生については言及されていない。抗菌薬投与とMRSA検出に有意の相関が認められている。
