Does microbial resistance or adaptation to biocides create a hazard in infection prevention and control?

医療関連感染症の症例数は、抗菌薬耐性菌による症例も含めて増加しており、感染制御のための消毒レジメンを改善する必要が生じている。結果的に殺生物薬を含む種々の抗菌薬に対する耐性に関する報告が増加している。殺生物薬と、その他の殺生物薬または抗菌薬との交差耐性については多くの報告がある。多くの文献では、殺生物薬への曝露を受けた微生物の順応と耐性とを明確に区別していない。順応は、殺生物薬を除去すれば元の状態に戻る点が耐性とは異なる。耐性は遺伝子に基づく現象であるため強力であり、容易には元に戻らない。このように明確な定義がないため、臨床の現場でこれらの現象の関連性を評価することが困難になっている。この総説では、順応プロセスおよび耐性の定義を提案するとともに、その定義に従って文献調査を実施した。結論としては、殺生物薬が適切に使用されていれば、殺生物薬耐性による医療へのリスクは現時点では低い。さらなる研究に取り組む必要がある。
