Invasive group B streptococcal disease in an orthopaedic unit

侵襲性B群レンサ球菌は整形外科患者の感染症の原因菌であり、増加が認められている。高齢、糖尿病、免疫不全などがリスク因子である。B群レンサ球菌は、化膿性関節炎（生体関節および人工関節周囲）、蜂巣炎、筋膜炎、軟部組織膿瘍、および骨髄炎の原因となり得る。本稿では、一般整形外科患者の大規模コホートにおけるB群レンサ球菌感染症の疫学と転帰について述べる。当院で3年間に深部組織または吸引検体にB群レンサ球菌を認めた全患者を特定し、カルテを後向きに調査した。対象患者は17例で、成人10万人あたりの1年間の発生率は0.69件、全緊急入院患者の有病率は0.12％であった。8例は人工関節関連であり、人工関節置換術後の発生率は0.15％であった。その他は骨折固定用金属製インプラント、骨髄炎、または慢性軟部組織感染症に関連していた。すべての分離株がベンジルペニシリン感性であったが、6株はマクロライド系抗菌薬に対して中等度の耐性を示した。16例は外科的創面切除を必要とし、2例は最終的に切断術が必要となった。本研究は、B群レンサ球菌は特定のリスク因子を有する患者の軟部組織および関節の感染症の原因菌となり得るという認識を強化するものである。
