Ward assessment of SmartIdeas Project: bringing source isolation to the patient

英国の病院の大半は、すべての感染患者を対象として感染源隔離を実施できるほどの十分な個室を備えていない。本研究の目的は、隔離システムの試作品を、専用に設計した可動式シンクおよびトイレと併せて一般病棟で評価することである。使いやすさと受容度に関する質問票を職員、患者、および訪問者に配布した。患者53例を隔離し、同時に環境サンプルの採取およびスタッフの手指衛生の観察を行った。感染患者の隣のベッドの閉鎖は行わなかったが、患者および職員は空間が狭いこととコミュニケーションに問題があると回答した。隔離ベッド空間への入室および退室時の手指衛生遵守率は有意に改善した［76回中43回（56.6％）から147回中107回（72.8％）へ、P ＜ 0.05］。トイレは好評であったが、機械的な信頼性に欠けていた。低レベルの微生物汚染（＜ 1～3.4 cfu/cm²）がすべての隔離ベッド空間に認められた。コロニー数は、最も接触の多い部位（リモートコントローラーなど）に最も多く認められた。メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）が、評価したすべてのシステムに同等のレベルで認められた。1つのシステムは空気隔離と接触隔離が可能なように設計されていたが、システムの内外からMRSAが検出されたことから、通気口の性能が不十分であったことが示唆される。これらの結果は英国ポートンダウンの健康保護局検査室（Health Protection Agency Laboratory）が空中微生物試験により確認した。ユニットを再設計した上で試験を行い、効果を確立することが必要である（Trial Identifier：ISRCTN02681602）。

本研究では3つの異なる形状の隔離システムを使用している。ガラス室のようなもの、ガラスの衝立、それに単なる衝立である。論文には写真が掲載されている。おのおの20例程度の使用経験に基づく結果が記述されている。こういった物理的障壁を設けると手指衛生の遵守率は少なくとも向上するようだ。しかし機械的な面などで問題点も浮かび上がっている。3つのシステムはいずれも有用なシステムと考えられ、個室の少ないイギリスだけでなく日本でも応用可能なシステムと思われる。さらなる検討と改善が必要である。
