Improving hand hygiene behaviour of nurses using action planning: a pilot study in the intensive care unit and surgical ward

病院の医師および看護師の手指衛生ガイドラインの遵守率は一般に低く、これを改善するためには、強力な社会科学的介入法を適用する必要がある。1つの方法は実行意図（implementation intention）（またはアクションプラン）の作成である。この方法では、状況的手がかり（environmental cue）を、意図的な行動による成果に結びつけるために、具体的な「if then」プラン^※を作成する。本パイロット研究では、大学病院のICUおよび外科病棟の看護師の手指衛生行動に対するアクションプランの実用性と効果を検討した。事前事後テストのデザインを採用し、看護師17名に参加を要請した。訓練を受けた調査者1名が、アクションプラン作成という介入の前および3週後に看護師の手指衛生行動を観察した。手指衛生行動の回数を計数し、ロジスティック回帰分析により手指衛生行動の変化を評価した。参加者17名中10名（外科病棟7名、ICU 3名）から完全なデータ得られ、この10名を解析対象とした。手指衛生を実施すべき機会が全体で283回確認され、このうち外科病棟142回、ICU 141回であった。手指衛生行動遵守率はベースライン時の9.3％から介入後は25.4％に増加した（オッズ比3.3、95％信頼区間1.7～6.5、P ＜ 0.001）。今回の研究は小規模ではあるが、その結果から、短期間で看護師の手指衛生行動を改善するうえでアクションプランの使用が有望であることが示された。アクションプランは他の分野では意図と行動の間のギャップの縮小に有効であることが示されており、医療環境での手指衛生行動を改善するためには、アクションプランの利用についてさらに調査する必要がある。
