Validation of intensive care unit-acquired infection surveillance in the Italian SPIN-UTI network

院内感染症サーベイランスでは、検証（validation）が最も重要な要素の1つである。本稿では、イタリアのNosocomial Infections Surveillance in Intensive Care Units（SPIN-UTI）プロジェクトのサーベイランスデータを対象とした初回の検証研究について報告する。その目的は、感染症データを検証し、SPIN-UTIネットワークに参加している集中治療室（ICU）の患者の院内感染症データの感度、特異度、陽性的中率（PPV）、および陰性的中率（NPV）を算出することである。検証研究はサーベイランス期間の終了時に実施した。訓練を受けた検証チームの医師が、すべての臨床的および検査室データが記載された全カルテを後向きにレビューし、PPV、NPV、感度、および特異度を算出した。SPIN-UTIに参加しているICU全49施設から8施設（16.3％）を無作為に選択し、検証を行った。検証チームは合計832例の患者（SPIN-UTI参加患者の27.3％）のカルテレビューを行った。PPVは83.5％、NPVは97.3％であった。全体の感度は82.3％、全体の特異度は97.2％であった。事前に訓練・指導を行っていたにもかかわらず、症例定義の誤解釈やプロトコールからの逸脱に関連する院内感染症の過大報告や過少報告がみられた。今回の研究の結果は、サーベイランスシステム内の方法論的な問題点の特定に有用であり、サーベイランスに携わる人材の再教育計画、およびSPIN-UTIプロジェクトの第2段階の計画と実践においても利用されている。
