Emergence of multi-resistant Pseudomonas aeruginosa in a Western Australian hospital

西オーストラリア州にある教育病院の患者から多剤耐性緑膿菌（Pseudomonas aeruginosa）が分離されて、発生数は2006年に初めて検出されて以降、引き続き増加している。2006年から2008年にかけて、多剤耐性緑膿菌保菌患者数は年間3例から9例に増加し、発生場所は集中治療室（ICU）から重症ケアユニット（HDU）へと移行した。職員の手洗い台に新しく設置した節水装置（エアレーター）が感染伝播に関与している可能性が指摘されて、手洗い台の配管のバイオフィルムから多剤耐性緑膿菌が分離された。手洗い台に廃棄された患者の体液、静注用抗菌薬の残り、および固形物が関与している可能性が考えられた。多剤耐性緑膿菌の遺伝子タイピングにより、他の病院の患者とは異なる遺伝子型系統が認められた。2008年の集団発生は長期間にわたり持続しており、環境に適応したことを示している。緑膿菌に対するより効果的な環境制御が早急に必要である。

監訳者コメント：
多剤耐性緑膿菌（P. aeruginosa）や多剤耐性アシネトバクター・バウマニー（Ａcinetobacter baumannii）の対策では、これらの菌が環境表面で生き延びることから、環境制御も重要となる。なお、緑膿菌の対策は水周りなど湿潤な環境を中心とするのに対して、A. baumanniiは乾燥した環境表面にも生存するために対策がより困難となる。
