Influence of job seniority, hand hygiene education, and patient-to-nurse ratio on hand disinfection compliance

2006年および2007年に、手指衛生遵守状況を直接観察により評価した。2007年には、直接観察の期間中に在職年数、手指衛生教育、および患者・看護師比などの特性のデータを収集した。2回の評価の間に手指衛生推進プログラムを実施した。単変量解析および多変量解析により、手指衛生遵守の改善に関連する因子を特定した。2006年から2007年に手指衛生機会は合計761回あり、完全遵守^※率は44.9％から58％に（P ＜ 0.001）、部分遵守^※※率は73.5％から88.4％に（P ＜ 0.001）改善した。2007年には、本調査対象の臨床部門に在職年数の長い医療従事者がいる場合は、手指衛生の完全遵守率（P ＝ 0.04）または部分遵守率（P ＝ 0.08）の改善が認められた。手指衛生の部分遵守率は、手指衛生教育実施後の2007年に有意に改善した（P ＜ 0.015）。観察時の患者・看護師比と遵守率との関連はみられなかった。多変量解析により、在職年数が手指衛生遵守の独立予測因子であることが判明した。今回の結果から、手指衛生遵守は手指衛生教育による影響を受けること、また在職年数の長い医療従事者は他の従事者へのロールモデルとなり得ることが示唆される。今後の衛生介入方法の策定にあたっては、これらのデータを考慮するべきである。
