Prospective assessment of two-stage testing for Clostridium difficile

一般的に使用されている免疫測定法は、クロストリジウム･ディフィシル（Clostridium difficile）感染症の診断のための単独の検査法としては限界がある。特に、この検査法の特異度は低く、偽陽性の結果が比較的多くみられる。著者らはC. difficile感染症の2段階検出法を日常検査に導入した。当検査室に送付された無形糞便検体をMeridian Premier酵素免疫測定法（EIA）による一次検査に供し、陽性検体には標準検査法（毒素産生性検査培養［toxigenic culture］および細胞毒性検査［cell cytotoxicity assay］）による再検査を実施した。臨床医に対しては検査室に検体が到着した当日に、診断に有用な情報としてEIAによる陰性確定または仮陽性の結果を提供した。無形糞便3,643検体で一次検査を実施し、このうち158検体（4.3％）がEIA仮陽性であった。EIA陽性の158検体中119検体は、少なくともいずれかの標準検査法で陽性と確認された。この集団の陽性的中率は75％（95％信頼区間67.6％～81.7％）であった。真陽性と偽陽性の検体のEIAの光学濃度（OD）を比較したところ、第2のカットオフ値を採用することにより診断を改善できることが示唆された。2つのカットオフ値を用いた検査では、「陽性」、「陰性」、「中間結果のため確認検査を要する」という結果が得られる。このアルゴリズムは、直ちに診断を改善できる簡便かつ費用対効果の高い方法であるが、検査室でのC. difficile感染診断のさらなる研究が急務である。

抗原検査と培養検査の併用をしないと、検査漏れがどうしても生じる。陽性検体の場合は、菌株保存をすることが望ましい。
