Cluster randomised controlled trial of an infection control education and training intervention programme focusing on meticillin-resistant Staphylococcus aureus in nursing homes for older people

このクラスター無作為化対照試験の目的は、感染制御教育・指導介入プログラムがナーシングホームにおけるメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）保菌率に及ぼす影響を調べることである。ナーシングホームを介入群（感染制御教育・指導プログラムを実施、16施設）、または対照群（従来の実践を継続、16施設）に無作為に割り付けた。介入群の施設職員には、良好な感染制御の実践の原則と遂行について教育・指導を行った（0、3、および6か月目）。全施設で監査を行い、良好な実践の遵守を評価した（0、3、6、および12か月目）。監査スコアおよび改善可能な実践を記載した報告書を、介入群のナーシングホームのマネージャーにフィードバックした。0、3、6、および12か月目に、同意を得た全入居者および職員の鼻腔スワブを採取した。主要評価項目は入居者および職員のMRSA保菌率、副次的評価項目は感染制御監査スコアの変化とした。全体で入居者793名および職員338名をベースライン時に登録した。試験実施期間中に、入居者および職員のMRSA保菌率に変化はみられなかった。12か月時点の介入群入居者のクラスタリング補正後のMRSA保菌相対リスクは、対照群との比較で0.99（95％信頼区間0.69～1.42）であった。12か月時点の感染制御監査スコアの平均値は、介入群（82％）のほうが対照群（64％）より有意に高かった（P ＜ 0.0001）。高齢者のナーシングホームでは、MRSAの減少に寄与し得る別のアプローチを考慮すべきである。
