Effects of gastric acidity on peristomal infection after percutaneous endoscopic gastrostomy placement

人工肛門周囲感染症は、経皮内視鏡的胃瘻造設術後に多くみられる合併症である。この研究では、胃内酸度が人工肛門周囲感染症に及ぼす影響（細菌の種類や、中咽頭の培養細菌と胃瘻チューブ部位の培養細菌との関連など）を調べた。1998年から2001年に、福井市の大滝病院で嚥下障害患者67例に経皮内視鏡的胃瘻造設術を実施した。胃内酸度を24時間pHモニタリングにより評価した。経皮内視鏡的胃瘻造設術後2週間、人工肛門周囲感染症の有無を観察し、中咽頭と胃瘻チューブ部位から検体を採取した。経皮内視鏡的胃瘻造設術施行患者の21例（31.3％）に人工肛門周囲感染症が認められた。中咽頭にメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の保菌が確認されなかった52例中11例（21.2％）に、人工肛門周囲感染症が認められた。胃内pH中央値は感染患者（11例）では5.05 ± 2.55（平均 ± SD）、非感染患者（41例）は3.06 ± 1.83であった（P ＝ 0.019）。MRSA保菌患者の66.7％（15例中10例）で人工肛門周囲感染症が認められたのに対して、MRSA非保菌患者では21.2％（52例中11例）であった（P ＜ 0.001）。人工肛門周囲感染症発生率は、胃内酸度や中咽頭のMRSAの有無による影響を受けていた。

人工肛門の造設部では、その内容物や宿主の保菌状況により周囲の皮膚の維持に大きく影響が及ぶ。パックや被覆材、テープなどの診療材料も同様に皮膚のコンディションを保つうえで重要な因子である。
