Respiratory syncytial virus outbreak defined by rapid screening in a neonatal intensive care unit

パリビズマブは現在、慢性肺疾患、早産、または血行動態的に重篤な先天性心疾患を有する乳幼児に対するRSウイルス下気道疾患の予防薬として承認されているが、新生児集中治療室（NICU）でのアウトブレイク時のルーチンの使用は、現時点では推奨されていない。本稿では、迅速抗原検査（Respi-Strip^R）を用いたRSウイルス感染スクリーニング試験実施中に発見されたNICUでのアウトブレイクについて報告する。2009年1月に、当NICUで検査を受けた早産児11例がRSウイルス陽性であった。その後、NICUの他の乳児の検査を行ったところ、さらに2例が無症候症例であることが判明した。交差感染の予防とともに、残りの早産児37例にパリビズマブの予防投与を実施した。投与2日後に、さらに2例にRSウイルスが検出され、症状が発現した。今回のアウトブレイクは、著者らの知る限り、NICUで発生し、迅速検査により早期に発見され、感染制御対策とパリビズマブの予防投与により効果的に制御された、最大規模のRSウイルスアウトブレイクである。

監訳者コメント：
RSウイルス感染症は、診断検査も治療・予防も日本の医療現場では一般的ではなく、比較的影の薄い病原体である。このような集団発生が今後増加することも予想される。
