壊死性筋膜炎は筋炎および筋壊死ととともに、皮膚・軟部組織感染症の病態の中で最も重度のものに位置づけられるが、丹毒、膿痂疹、および蜂巣炎とは大きく異なっている。特にレンサ球菌性壊死性筋膜炎に由来する外毒素やスーパー抗原が介在する合併症は、多様な感染所見を呈するため、経験の乏しい臨床医は判断を誤りやすい。生存率を改善するためには臨床所見から早期に疾患を疑い手術を施行することが重要であり、壊死性筋膜炎患者に対しては病原体、感染部位、および産生毒素の作用に応じた総合的な集学的管理が必要である。種々の臨床的パラメータおよび臨床検査パラメータを組み入れた多変量的アプローチは、積極的な管理の一助となると考えられる。本総説では主要なタイプの壊死性筋膜炎の診断と管理について述べ、過去の研究や適切な診断法の中から病因解明の重要な糸口を明らかにする。また、高圧酸素療法や免疫グロブリン静注療法などの検討が進められている治療法について、潜在的な有益性を考察する。
