Age as an independent risk factor for surgical site infections in a large gastrointestinal surgery cohort in Japan

消化管手術を受ける日本人患者の手術部位感染症（SSI）のリスク因子を評価するために、大規模調査を実施した。研究の目的としては、（i）消化管手術によるSSIのリスク因子として年齢を検討すること、および（ii）腹腔鏡下胆嚢摘出術と開腹胆嚢摘出術における SSIのリスク因子の相違を検討することとした。2003年7月から2007年11月に日本の参加病院20施設から調査データを前向きに収集した。SSIの診断は米国疾病管理予防センター（CDC）の基準を使用した。SSIはデータが得られた12,015例中1,471例に認められ、全発生率は12.2％であった。最終的なロジスティック回帰モデルにおいては、年齢は開腹胆嚢摘出術、胃切除術、および虫垂切除術でSSIリスク因子であった。手術時間は6種類の手術手技でのSSIのリスク因子であり、創分類およびドレーンの使用も半数以上の手術手技でのリスク因子であった。腹腔鏡下手術を開腹手術と比較すると、絹製縫合糸の使用は腹腔鏡下胆嚢摘出術でのSSIリスク因子であった。ドレーンの使用、創分類、手術時間、男性であること、および年齢は、開腹胆嚢摘出術でのSSIリスク因子であった。まとめると、患者年齢はいくつかの消化管手術でのSSIの有意な予測因子であるが、腹腔鏡下手術でのSSIのリスク因子は開腹手術の場合とは大幅に異なると考えられる。
