Surveillance of selected post-caesarean infections based on electronic registries: validation study including post-discharge infections

手術後の入院期間が短縮していることから、退院後感染症のサーベイランスの重要性が高まっている。本研究では、帝王切開後30日以内に発生した尿路感染症および手術創感染症を対象とした、コンピューターによるサーベイランスシステムの検出能を検討した。参照基準に従って分類した帝王切開後尿路感染症および手術創感染症患者を特定することを目的として、種々の電子記録データの利用について評価した。参照基準は、カルテおよび患者報告データ（質問票）の情報に基づき、米国疾病対策センターの定義を修正し作成した。入院中、再入院中、または外来受診時に診断された尿路感染症の、コンピューターシステムによる検出感度は80.0％、特異度は99.9％であった。退院後に院外で診断された尿路感染症の感度は76.3％、特異度は99.9％であった。院内で診断された手術創感染症および退院後に院外で診断された手術創感染の感度はそれぞれ77.1％、68.9％、特異度は99.5％、98.2％であった。結論として、コンピューターを用いたサーベイランスシステムにより院内感染症と退院後感染症を特定できる可能性があり、その感度は比較的高く、特異度は極めて優れていた。

監訳者コメント：
我が国は平均在院日数が長く、手術部位感染症（SSI）の調査はむしろ、より正確な把握がなされている可能性がある。日帰り手術の増加とともに、自宅でのSSI事例のモニタリングが課題である。
