手術部位感染症(SSI)は腹部手術後に一般的な合併症であり、その診断とサーベイランスには一般的に米国疾病対策センター(CDC)の基準が用いられる。この研究の目的は、CDC基準に従って診断したSSIが臨床的に重要であるか否か、およびCDC基準に基づく評価とASEPSIS(Additional treatment[追加治療]、presence of Serous discharge[滲出液]、Erythema[紅斑]、Purulent exudate[膿性滲出]、Separation of the deep tissues[深部組織の離開]、Isolation of bacteria[細菌の分離]、duration of Stay[入院期間])スコア、および臨床的に重要なSSIとの間に一致がみられるかどうかを検討することであった。開腹手術後にCDC基準に基づいてSSIと診断された患者54例と、SSIと診断されなかったマッチさせた対照46例を対象とした。熟達した外科医2名が無作為にカルテを評価して、以下の基準に従って患者が臨床的に重要なSSIであるか否かを判定した。その基準は、抗菌薬治療、外科的介入、入院期間の延長、またはSSIによる集中治療部門への入室であった。臨床的に重要なSSIは、CDC基準に基づいてSSIと診断された患者で54例中38例(70%)に認められたが、CDC基準で SSI と診断されなかった患者には認められなかった。CDC基準に基づいて診断されたSSIの61%が表在性であり、その中では48%が臨床的に重要であると判定された。CDC基準と臨床的に重要なSSIの一致度は高く(κ = 0.69、95%信頼区間[CI]0.55~0.83)、ASEPSISスコアとCDC基準の一致度(κ = 0.23,95%CI 0~0.49)およびASEPSISスコアと臨床的に重要なSSIの一致度(κ = 0.39,95%CI 0.17~0.61)は高くなかった。CDC基準からは、一部に臨床的重要性の低い表在性SSIが含まれるとしても、SSIの監視と同定に適した標準的定義を得ることができる。

監訳者コメント
サーベイランス診断は、サーベイランスの本来的な目的である継続的な質改善のための基準であり、一般的には臨床診断と必ずしも一致する必要はないと考えられているが、この論文ではSSIのCDC基準が臨床的にも有用であることを示している。SSIサーベイランスへの外科医の協力を得るにはよい材料かもしれない。
