Prevalence of hospital-acquired infections and antibiotic use in two tertiary Mongolian hospitals

モンゴルの保健統計によると、病院感染は全入院の0.01％から0.05％で発生している。これは、世界各国で報告されている数値より相当低い。そこで2008年9月にウランバートルにある2つの3次病院で1日調査を実施し、病院感染症有病率、関連リスク因子、および抗菌薬使用パターンの評価を行った。調査対象患者933例中50例（5.4％）が病院感染症と診断された。有病率は手術部位感染症1.1％（手術患者の3.9％）、血流感染症0.3％、呼吸器感染症1.3％、尿路感染症1.3％、その他の病院感染症1.4％であった。微生物学的検査の記録があったのは、全患者の18.9％のみであった。合計558例の患者（59.8％）が、902コースの抗菌薬治療を受けていた。このうち92.1％は、感受性試験を実施せずに抗菌薬を処方されていた。多重ロジスティック回帰分析により、病院感染症と有意な関連が認められた因子は、入院前の滞在場所、病院、入院期間、手術やその他の侵襲的処置、尿路カテーテル、およびその他の留置デバイスであった。本研究の結果は他の発展途上国の報告と同等であり、公式統計はモンゴルにおける病院感染症の真の頻度を過小評価していることが確認された。
