スフィンゴモナス・パウシモビリス（Sphingomonas paucimobilis）：持続的な院内感染グラム陰性菌★

ブドウ糖非発酵グラム陰性桿菌は、極めて広範に認められる院内感染症の起因菌であり、臨床環境に重大な問題を引き起こしている。これらは平素は無害病原体（監訳者註：いわゆる日和見病原体）であり、患者の基礎状態や基礎疾患によっては感染症を生じる。ブドウ糖非発酵グラム陰性桿菌のスフィンゴモナス・パウシモビリス（Sphingomonas paucimobilis）は、臨床的な重要性は小さいと考えられているが、文献的にはこの菌による多数の感染例が報告されている。これらの感染症には蒸留水、血液透析液、滅菌薬液などの汚染溶液によって生じる菌血症・敗血症がある。S. paucimobilisに関連しては偽性菌血症の症例報告があるが、一方では、細菌性関節炎や骨髄炎などのまれな重度かつ侵襲性の感染症症例も少なくない。S. paucimobilisに関連する文献の中に死亡例は認められなかった。この総説では、S. paucimobilisがこれまで考えられていたよりも重要な病原体であることを示す。

スフィンゴモナス・パウシモビリス（Sphingomonas paucimobilis）は、様々な環境に広く認められるブドウ糖非発酵グラム陰性桿菌であり、かつては宇宙船ミールの中でも認められたことがあるような適応性にも富んだ細菌である。医療環境に由来する病原体細菌としては、緑膿菌（Pseudomonas aeruginosa）やアシネトバクター属（Acinetobacter spp.）などのブドウ糖非発酵グラム陰性桿菌、腸内細菌科に属するセラチア属（Serratia spp.）、芽胞産生性であるクロストリジウム・ディフィシル（Clostridium difficile）やバシルス・セレウス（Bacillus cereus）などがあり、感染防止対策としても医療環境整備は重要である。
