Efficacy of a standard meticillin-resistant Staphylococcus aureus decolonisation protocol in routine clinical practice

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）保菌者の除菌により、MRSA感染症の発症リスクが低下し、他の患者への伝播が予防できると考えられる。今回の前向きコホート研究の目的は、標準的な除菌レジメンの長期効果を明らかにするとともに、除菌の不成功に関連する因子を特定することである。MRSA保菌患者に除菌を実施し、スクリーニングとして治療後に週1回、特定の部位からスワブを採取して、3回連続で増殖が認められない場合を除菌の成功と見なした。除菌が成功した患者に対しては、追跡培養検査を6か月後と12か月後に実施した。登録患者137例中79例（58％）で除菌が成功した。この79例中53例（67％）は6か月後、44例（56％）は12か月後に除菌が維持されていた。したがって、12か月後にもMRSA陰性であったのは、除菌を完了した137例中44例（32％）のみであった。これらの成績が特定のMRSA株に関連することはなかった。除菌成功率が低かったのは、ムピロシン耐性菌株保菌患者（補正オッズ比［OR］0.08、95％信頼区間［CI］0.02～0.30）、咽頭に保菌を認める患者（補正OR 0.22、95％CI 0.07～0.68）、および60歳から80歳の患者と比較した80歳を超える患者（補正OR 0.30、95％CI 0.10～0.93）であった。これらの知見から、今回使用したプロトコールにより除菌に成功する例はみられるものの、長期的には半数以上の患者でMRSA除菌を達成できないことが示唆された。

MRSA保菌者のスクリーニングに関しては様々な研究や意見が出されているが、除菌の効果については知見が少ない。本論文は除菌後の追跡調査を行うことにより、除菌不成功の高リスク患者を同定し、除菌を効率よく実施するための一助となる可能性がある。
