Speed of molecular detection techniques for meticillin-resistant Staphylococcus aureus admission screening in an acute care hospital

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）保菌者の積極的監視は、急性期病院におけるMRSA制御対策に不可欠な要素と考えられている。近年、MRSAスクリーニングに対してサンプル処理時間が大幅に短縮される分子アッセイが利用可能となっている。時間分析モデルを用いて分子アッセイ導入後の時間節約を調査し、従来の培養技術による時間を対照として比較した。4か月の試験期間中、高リスク患者（44例）およびMRSA陽性が判明している再入院患者（41例）すべてを対象として、入院時のMRSAスクリーニングを実施した。両群ともに、全処理過程の中で検査までの時間（入院から、検体採取または検体の臨床検査室への搬送までに要した時間）が長いことが律速要因になっていた。分子アッセイを使用すると、従来の培養法と比較してサンプル処理時間が大幅に短縮した。検査までの時間が長いことに加えてPCR法が高価であっため、分子アッセイは入院時のスクリーニングに導入されなかった。しかし、再入院患者群のスクリーニングでは、検査結果が早期に判明したことによって不要な隔離日数が大幅に減少し、結果的に病院に経済的利益をもたらした。PCR法は再入院時のスクリーニングには有益である可能性がある。結論として、高価なPCR法を導入する前に、病院のMRSAスクリーニングに関する施策を検討すべきである。

監訳者コメント：
本論文はベルギーの医療施設で実施されたMRSAスクリーニングの検査法に関する研究である。MRSA陽性患者だけでなく疑いの濃い症例に対して隔離予防策を実施する状況では、高価なPCR法によるスクリーニングがMRSA陰性患者の早期隔離解除につながり、費用対効果的に優れている可能性がある。しかしそのような指針を取っている日本の医療施設は多くないと思われ、本法は費用対効果的に優れているとはいえない可能性が高い。本法は迅速に結果を得ることができるが、その普及にはまだまだ様々な問題をクリアする必要があるだろう。
