アイルランドの保健児童省(Department of Health and Children)は、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)高リスク患者に対する入院時スクリーニング、これらの患者のMRSA陰性確定までの隔離、および同定されたすべてのMRSAの根絶を推奨している。これらの対策は、スカンジナビアでMRSAの減少に成功している「search and destroy」と呼ばれるプログラムの基盤となっている。しかし、アイルランドではsearch and destroyの実施に関して公表されている情報はほとんどない。本研究は、定量的な準実験的デザインを用いた介入コホート研究として実施した。一病院を対象として、確立しているMRSA入院時スクリーニングプログラムを用いて、MRSAスクリーニング検査結果が出るまでの所要時間の短縮(2007年)および暫定的隔離の導入(2008年)への効果を評価した。介入後にMRSA感染・保菌率を調査し、介入前(2005~2006年)のベースライン値と比較した。病院感染(院内感染)MRSA感染・保菌率は、2007年および2008年のいずれも低下していた。しかし、本研究は準実験的デザインを採用していること、およびこの病院のMRSA流行レベルが低いことから、その因果関係を示すことはできなかった。

監訳者コメント
Search & Destroy (S&D) policyは、オランダ、スカンジナビア半島の北欧諸国で成功を収めている対策であるが、徹底したMRSAの捜索と除菌によりMRSAの総数を減らそうという考え方である。これを実施するには莫大な費用と手間がかかるので、米国や日本のようにすでに蔓延してしまっている国ではとてもまかない切れない。しかしながら、高リスクな患者に絞ってスクリーニングを行い、それ以外の保菌者については手指衛生と標準予防策で伝播リスクを最小化しようという考え方は、従来のrisk assessment(RA)policyに加え、S&Dの良い点を融合させた改良型RAとして今後の普及と発展が期待される
