Epidemiology and clonality of multidrug-resistant Acinetobacter baumannii from a healthcare region in Hong Kong

香港における多剤耐性アシネトバクター・バウマニー(multidrug-resistant Acinetobacter baumannii;MDR-AB)のリスク因子および分子疫学を評価した。2005年から2006年に、同一医療圏(healthcare region)の5つの病院で治療を受けた患者を対象とした。増幅リボソームDNA制限解析(ARDRA)によるゲノム解析を行い、表現型解析法および分子型解析法によりメタロβ-ラクタマーゼの存在および代表的なMDR-AB菌株のクローン性を調べた。MDR-ABが分離された45例を、対照(MDR-ABが分離されなかった患者)135例と比較した。ロジスティック回帰分析により、MDR-AB保菌・感染と関連する独立因子は慢性創(オッズ比29.5、95%信頼区間8.1縲・07.2、P < 0.001)のみであることが示された。ARDRAでは、MDR-AB分離株45株すべてが遺伝子種2TUであった。パルスフィールド・ゲル電気泳動法により、分離株2株を除くすべての分離株が2つのクローン型に分類され、内訳はHKU1が24株、HKU2が19株であった。HKU1株の主な特性はST26、adeB XII型、blaOxA-23-like遺伝子およびblaOxA-51-like遺伝子陽性、およびカルバペネム高度耐性であった。HKU1株の多くはゲンタマイシン、cotrimoxazole、およびミノサイクリンに感性であった。一方、HKU2株はST22、adeB II型を示し、ほとんどはblaOxA-51-like遺伝子のみ陽性で、ゲンタマイシン、cotrimoxazole、およびミノサイクリンに耐性であった。いずれのクローンも広く拡散していることが判明した。結論として、クローンの拡散が、香港のこれらの病院におけるMDR-AB増加の主な原因となっている。今回の知見は、感染制御対策の強化の必要性を示している。

監訳者コメント:
日本でも多剤耐性アシネトバクターが時々出現しているが、本論文のように地域に拡散している状況には、幸いにしてまだない。他山の石とすべき近隣諸国の状況を示した文献である。
