Routine antibiotic use in preterm neonates: a randomised controlled trial

早産新生児は免疫系が未熟であるため、新生児敗血症のリスクが高い。ルーチンの抗菌薬投与による早産新生児の臨床的敗血症発生への効果を比較するため、部分盲検無作為化対照試験を実施した。生後12時間以内に入院した、感染症に関連するその他のリスク因子がみられない早産新生児を、抗菌薬をルーチンに投与する群と対照群(臨床的適用がない限り抗菌薬を使用しない)に無作為に割り付けた。主要評価項目は臨床的敗血症発生率とした。副次的評価項目は血液培養陽性率、ステージII/IIIの壊死性腸炎発生率、死亡率、および入院期間とした。臨床的敗血症の発生率は両群で同等であった(介入群31.9％、対照群25.4％、 P ＝ 0.392)。死亡率は両群で同等であった。対照群は血液培養陽性率が有意に高かった(P ＝ 0.002)。壊死性腸炎の発生率および入院期間は両群で同等であった。低リスクの早産新生児では、ルーチンの抗菌薬投与に防御効果があるとするエビデンスは得られなかった。

ルーチンで抗菌薬投与を行うというまったくナンセンスなプラクティスの無意味さを証明したよい論文である。
