Use of octenidine dihydrochloride in meticillin resistant Staphylococcus aureus decolonization regimens: a literature review

メチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）（MRSA）保菌患者の除菌は、病院内でMRSAを制圧する方法の1つとして推奨されているが、使用できる薬剤の選択肢は限られている。オクテニジン二塩酸塩は比較的新しい消毒薬であり、一部の国々でMRSA除菌に利用されている。MRSA除菌に対するこの薬剤の利用について入手が可能な文献をレビューしたところ、4件の観察研究しか見いだされなかった。これらはいずれも小規模な研究であり、研究デザインは異なっていた。MRSA除菌率は6％から75％と報告されていた。これらの研究には創部保菌患者は含まれていたが、このうち2件の研究では体毛の処置について記載がなかった。オクテニジンのMRSA除菌効果はクロルヘキシジンと同等であり、有害事象はより少ないと考えられたが、臨床環境での有用性を確認するには、MRSA除菌のための皮膚消毒薬としてオクテニジンを組み入れた大規模無作為化臨床試験を行う必要がある。

監訳者コメント：
臨床的にMRSA除菌の選択肢は限られており、わが国においては鼻腔内ムピロシン投与による方法しかない。まだ日本でオクテニジンは利用されていないが、今後の展開を注目したい。
