外科患者の周術期の酸化活性の増加は、術後の手術部位感染(SSI)を予防するとする報告がある。いくつかの臨床研究から、セボフルラン麻酔下の酸化活性はプロポフォール麻酔下と比較して高いことが示されている。そこで著者らは、セボフルラン麻酔はプロポフォール麻酔と比べてSSIを抑制するという仮説を立てた。SSIに対するセボフルランおよびプロポフォールによる麻酔維持の効果を検討するために、2007年1月から2008年12月に全身麻酔下で消化管の待機的開腹手術を受けた、米国麻酔科医学会全身状態スコア1~3の成人連続症例265例を対象としてSSIのサーベイランスを行った。麻酔維持として95例にセボフルラン、170例にプロポフォールを選択した。2種類の麻酔方法間の選択バイアスを回避するために、傾向スコア解析によりこれらの患者のペアワイズマッチングを行い、患者84組を決定した。セボフルラン麻酔群とプロポフォール麻酔群の間で、標準化感染比、すなわち実際のSSI発生数と全米病院感染サーベイランス(NNIS)のデータを用いて算出した予測されるSSI発生数の比率を比較した。傾向マッチング実施後のセボフルラン麻酔群の標準化感染比は1.89[95%信頼区間(CI)1.46~2.32]で、プロポフォール麻酔群(4.78、95%CI 4.30~5.27)と比較して有意に低かった(P = 0.02)。本研究から、セボフルランはプロポフォールと比較して消化管の待機的開腹手術後のSSIを抑制する傾向があることが示唆された。

監訳者コメント
各群の症例数が100例未満と少ないが、マッチングがよく検討されている2群間での比較であり、この差はセボフルランによる麻酔のSSI抑止効果を示していると言えるのではないか。日本からのスタディであり、大規模な追試が待たれる。
