手術時手指消毒は、手術による感染を減少させる手段の1つとして、19世紀から推奨されてきた。本論文では、手術時手指消毒のエビデンスおよび主要目的と、現在使用されている製品の選択基準についてレビューした。北米と欧州の手術時手指消毒用製品を選択する際の試験・検証法を比較した。薬用石けんおよび擦式アルコール製剤を用いた手術時手指消毒について、その技術面、処置に要する時間、乾燥時間、想定される副作用、および最適な製剤選択のための因子などの考察を行った。手術時手指消毒にブラシは推奨されない。抗菌作用の即効性、広範な抗菌スペクトル、副作用が少ないこと、およびすすぎ水による手指汚染リスクがないことから、手術時手指消毒には擦式アルコール製剤の使用が望ましいことは明らかである。このことは、水供給が乏しいか水質に問題があるような資源の限られた国にも当てはまる。

監訳者コメント：
アルコールは確かに優れた手指消毒薬であるが、たとえ手術直前に実施しても、手指表面の常在菌が死滅するだけで、毛嚢内の汗腺に生息しているブドウ球菌などは発汗に伴って表皮上に分泌されてくる。よって、クロルヘキシジンなど残留活性がある消毒薬を混合させた製剤がより望ましいと考えられてきたが、明確なエビデンスがないためにここでは単にアルコール製剤のみで括られている。State-of-artという割には、ツッコミが少々甘い感じのレビューである。
