Healthcare-associated infection and the patient experience: a qualitative study using patient interviews

患者の声が耳に届くようにするためには、医療への患者の関与を増大させる必要があることが重視されてきている。今回の探索的研究では、患者との詳細な対面面接を実施して、医療関連感染症に関する体験を聴取した。面接の対象は、黄色ブドウ球菌（Staphylococcus aureus）血流感染症と診断された患者および同一の病院に入院したが血流感染症と診断されなかった患者であった。感染症の有無にかかわらず、ほとんどの患者が重大な懸念事項として挙げたのは、コミュニケーションが口頭と書面のいずれも不足していたことであった。また、一部の患者は質問することにためらいを感じたと述べ、職員に対してその診療内容を質したいと述べた患者やその家族はごく少数であった。一部の患者は国民保健サービス（National Health Service；NHS）に継続的な信頼を寄せていたが、大多数の患者は医療関連感染症に関してNHSをほとんど、または全く信用しておらず、再び入院した場合には医療関連感染症が深刻に懸念されると述べた。これらの結果から、感染の予防と制御に関して、医療の質や患者の安全の向上を図り、患者の体験を改善するためには、取り組むべき多くの課題があることが示唆される。また、政策立案者、病院管理者、およびすべての医療従事者は、制度変更や情報に関する計画・評価への患者の関与を強化すべきである。
