緑膿菌(Pseudomonas aeruginosa)は多様な感染像を示す病原体であり、ヒトの広範な感染症に関与している。緑膿菌は、医療現場においては、熱傷や好中球減少症、または集中治療在室中などの脆弱な患者に対する、重要な感染症の原因の1つである。このような集団では、緑膿菌感染症による罹患率と死亡率が高い可能性がある。緑膿菌は多くの抗菌薬に耐性を示すので、感染管理が難しい。さらに、治療選択肢として残されている数少ない薬剤に対する耐性が出現し拡大しているため、治療はますます困難になっている。近年の注目すべき現象として、一部の緑膿菌株にみられるカルバペネマーゼの獲得が挙げられる。これらの問題に関しては、入院患者の菌獲得を予防するための戦略を明確にすることが妥当と思われる。緑膿菌の環境リザーバの特定は容易であり、環境中の感染源からのアウトブレイクに関する報告は多数あるが、散発的な緑膿菌感染症における環境ソースの役割はあまり解明されていない。しかし、集中治療室などの医療環境での散発的な緑膿菌感染の疫学においては、特に水をはじめとする環境ソースが重要であることが、前向き研究による新たなエビデンスから示唆されている。緑膿菌感染における環境リザーバの役割の理解を深めることにより、これらの感染源から患者への感染を防ぐための新たな戦略の開発と現行アプローチの改善が可能となるであろう。

監訳者コメント:
多剤耐性緑膿菌(MDRP)などのuntreatable infectionの管理においては、抗菌薬の適正処方だけではその拡大を抑止することは困難である。なぜなら、メタロβ-ラクタマーゼなどの耐性遺伝子はプラスミド上にコードされており、菌種を超えてプラスミドの授受を行っているからである。抗菌薬適正化と同時に、環境リザーバの駆逐を行うことが多剤耐性遺伝子を環境から駆逐することにつながるのである。
