The role of environmental cleaning in the control of hospital-acquired infection

この10年間、病院感染の増加が大きな注目を集めている。一般市民はいわゆる「スーパーバグ（superbug）」を、汚れた病院に関して自分たちの見聞した経験と結び付けてきたが、これらの微生物の制御における環境清掃の正確な役割は依然として不明である。清掃がエビデンスに基づく科学として認められ、その評価法が確立するまでは、清潔な環境の重要性は推論の域を出ないものと考えられる。本総説では、病院環境と種々の病原体、すなわちメチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）、バンコマイシン耐性腸球菌、ノロウイルス、クロストリジウム・ディフィシル（Clostridium difficile）、およびアシネトバクター（Acinetobacter）属菌などとの関連を検討する。これらの微生物は医療環境で生存する能力があるが、清掃過程で脆弱化することを支持するエビデンスが得られている。消毒薬の使用の有無にかかわらず、病原体の除去は患者への感染率の低下に関連すると考えられる。残念ながら、清掃はアウトブレイクに対する感染制御対策の一環として実施されることが多く、単独の介入としての重要性には依然として議論がある。手の触れる場所は常に病院感染病原体に汚染されており、その病原体は患者の手指に伝播することが最近の研究で示されている。手の触れる箇所に関する詳細な研究は少ないため、これらの箇所の清掃を重点的に行うことは、現在重視されている手指衛生にも効果的に寄与する可能性がある。さらに、提唱されている病院衛生基準を用いることによって、清掃は病院感染制御のための費用対効果に優れた介入であることのエビデンスをさらに提示することができると考えられる。

環境清掃の効果と意義に関する総説。冒頭に、その正確な役割は依然として不明であるとあるが、同時に本総説においては利用可能なエビデンスを提供している。環境清掃を考える上で手元に置いておきたい論文である。
