感染予防・制御対策を有効なものとするには、これらを病院運営組織における複雑で重層的なシステム^※の連携の中に組み入れる必要がある。各システムに関係する活動をどのように至適化すれば患者への感染リスクが最小になるかを、システムごとに検討する必要がある。本稿では、質の保証および患者の安全に関する目標を達成するために必要となる、病院組織の構成要素について論じる。また、感染制御基準に関連するパフォーマンス・マネジメントツールの利用についてレビューするとともに、システム管理の欠陥の代用指標として病院感染を使用することについて考察した。感染予防・制御は個人または少人数の専門チームの役割や責任で対応するのではなく、あらゆるレベルで優先すべきものであり、研究や教育計画を含むすべての運営システムの中に組み入れる必要がある。
