Controversies in infection: infection control or antibiotic stewardship to control healthcare-acquired infection?

医療関連感染の制御に関する情報資源は豊富であるにもかかわらず、その発生率はかつてないほど高い。Alexander Gordonによる産褥敗血症が伝染性であることの発見は、細菌学および抗菌薬の黄金時代の到来を告げたが、このことは感染症が撲滅されたという信仰を招来し、次いで我々を誤った安全意識と抗菌薬に対する過度の依存へと導いた。現代医療は、抗菌薬を用いることで多大な進歩を遂げてきたが、その成功自体が甚大な過量投与をもたらし、薬剤耐性の問題に至った。抗菌薬の適切なパイプラインが枯渇していることと併せて、我々は現在、抗菌薬の矛盾－すなわち、抗菌薬が実際に多くの医療関連感染症を引き起こしているかもしれないということ－に対処せざるを得ない状況に陥っている。クロストリジウム・ディフィシル（Clostridium difficile）感染のみならず、メチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）感染をはじめとする多くの感染症は、程度の差はあるものの抗菌薬処方に完全に依存している。我々が医療関連感染に立ち向かう方法として、処方の管理は従来の感染制御対策と同等の有効性を有すると思われる。従来の感染制御が「消火活動」であるとすれば、抗菌薬使用の管理は「防火活動」といえる。

監訳者コメント：
感染制御の有効性と抗菌薬の管理の有効性を比較した総説というよりは、抗菌薬の管理が感染制御にどれほど役立つかに関して述べた総説であり、多分に筆者の考えが反映されている。しかし、一読の価値はある。
