本研究では、院内感染に関するドイツの病院サーベイランスシステム(Krankenhaus Infektions Surveillance System;KISS)から得られた、人工呼吸器の使用および人工呼吸器関連肺炎(VAP)の発生率に関するデータを示す。2005年に米国疾病対策センター(CDC)の新しい定義が使用できるようになった。データが異常な範囲にある集中治療室(ICU)の特定にあたって、KISSが用いたこの新たな方法について報告する。1,000人工呼吸器使用日あたりのVAP症例数を算出し、異常値を特定するために新たな視覚的方法である漏斗プロット法を導入した。少ない人工呼吸器使用日数で観察されたVAP症例が少数の場合は、その発生率は偶然のばらつきに大きく影響されることが予想される。漏斗プロット法では、このようなイベント発生率と症例数との関係が考慮される。2005年1月から2007年12月までの期間に886,816例のサーベイランスデータが合計391のICUから報告され、VAPは6,896例、延べ3,113,983患者・日であった。CDCの新たな定義に従った平均VAP発生率は、1,000人工呼吸器使用日あたり5.5例であった(中央値4.4例)。全ICUの人工呼吸器の平均使用率は35.7%(中央値29.3%)であった。漏斗プロット法により異常値と特定されたICUは14.3%であり、その内訳は異常高値34施設、異常低値22施設であった。2008年以降、KISSの参加ICUでは漏斗プロット法による視覚的フィードバックが実施されている。漏斗プロット法はヒストグラムと比較して誤解釈が少なく、VAP増加に対する調査を実施すべきタイミングを示してくれる。
