Regional trends in multidrug-resistant infections in German intensive care units: a real-time model for epidemiological monitoring and analysis

ドイツ院内感染サーベイランシステム（German Nosocomial Infection Surveillance System）の集中治療の構成要素として、多剤耐性菌を対象とした新たなサーベイランスモジュールが加えられた。参加する集中治療室（ICU）はメチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）（MRSA）、バンコマイシン耐性腸球菌（VRE）、および基質特異性拡張型β-ラクタマーゼ産生大腸菌（Escherichia coli）・肺炎桿菌（Klebsiella pneumoniae）（ESBL-EC/KP）の保菌・感染患者全員のデータを報告する。ドイツ国内におけるMRSA、VREおよびESBL-EC/KPの地域的分布を明らかにするため、2005年度と2006年度の各ICUにおけるこれらの細菌の1,000患者・日あたりの保菌密度（incidence density）を算出し、国内5地域のICU情報として蓄積した。合計176のICUの284,142例、延べ1,021,579患者・日から、多剤耐性菌症例5,490例のデータが報告された。蓄積された1,000患者・日あたりの保菌率はMRSA 4.54例、ESBL-EC/KP 0.54例、VRE 0.29例、1,000患者・日あたりの感染率はMRSA 1.56例、ESBL-EC/KP 0.32例、VRE 0.13例であった。MRSA感染率に有意な地域差は認められなかったが、VREおよびESBL-EC/KPの感染率には地域間に有意なばらつきがみられた。2005年から2006年にMRSA、ESBL-EC/KP、またはVRE感染を1件以上報告したICUの割合にも地域差がみられ、その範囲はMRSA 82％～91％、ESBL-EC/KP 34％～76％、VRE 8％～42％であった。この新たなサーベイランスモジュールにより、ICUが地域の保菌密度と国の基準との比較を通じて多剤耐性菌の発生をモニタリングすることが可能となり、ドイツでは多剤耐性菌に地域間の有意なばらつきがあることが示された。

監訳者コメント：
ドイツ国内の地域間における耐性菌のターゲットサーベイランスをICUで行った報告である。欧州でも国により耐性菌の概況が異なることが分かっており、それらは隣接する国家間の影響があるのかが興味深い。
