Survey of teaching/learning of healthcare-associated infections in UK and Irish medical schools

すべての医師は、医療関連感染症の診断、管理、および予防に重要な役割を果たしている。医師と医学生の医療関連感染症予防プログラムへの寄与を強化するには、知識の向上、技術の改善、および患者ケアの安全性と質に対する適切な姿勢を身につけることが必要である。Hospital Infection Society（HIS）の資金援助により、英国およびアイルランド共和国のすべての医科大学を対象として医学教育に関する調査を行った。質問票を作成してすべての医科大学に配布し、38校中31校（82％）から回答を得た。医療関連感染症の有病率および伝播について教えている医科大学はそれぞれ97％、100％であったが、医療の質と安全性の問題としての医療関連感染症の重要性を教えているのは60％のみであった。医学生を評価する方法としては、多肢選択問題（MCQ）と客観的臨床能力試験（OSCE）の採用率が高かった。講義、症例検討、および実技デモンストレーションを有益と考えている医科大学は90％を超え、オンライン教材は67％、業務日誌は60％が有益性を認めていた。蓄積された共通の教育資源を共有することに前向きなのは80％以上であった。最適な評価法についての概要をまとめ、蓄積された教育資源を共有するためには、医療関連感染症の予防と制御に関する医学生指導用カリキュラムを合意を得て作成する必要がある。

英国では、感染制御部門の部長をDirector of Infection Prevention and Control（DIPC）として各病院に配置が義務付けられており、患者安全委員会（Patient Safety Committee）への出席が義務付けられている。これは感染制御が病院の収益向上云々の問題からではなく、医療安全と質の観点から患者の権利を保護する意味合いが、社会的により強くなっていることを反映している。我が国も正しい方向で患者の権利を守る機運が高まることを期待している。
