Role of interferon-gamma release assays in healthcare workers

新規に開発されたインターフェロンγ放出測定法（interferon-gamma release assays；IGRA）は、潜伏性結核感染検出法の新たな選択肢となっている。これは、感染リスクが高いがワクチン接種率も高い医療従事者にとって、大きなかかわりがある。本論文では、種々の医療環境の潜伏性結核感染診断におけるIGRAの意義を検討する。医療従事者を対象としたIGRAのデータを報告している過去18年間の研究を検索し、22件が対象基準に合致した。IGRAは、結核発生率が高い国を除くとツベルクリン反応検査（TST）との一致率は低かったが、接触者調査も含めて結核曝露マーカーとの相関は概ね良好であった。T-SPOT^R.TB法の評価を医療従事者を対象として行った研究は少なく、わずかな研究からはT-SPOT^R.TB法の特異度はTSTと比較して高いことが示されている。本総説から、IGRAは医療環境での結核予防および管理の重要な手法として利用できることが明らかになった。

医療現場におけるインターフェロンγ試験の有用性をまとめた総説。ツベルクリン反応試験との詳細な比較も行われており、有用な論文である。なお、専門的知識がないと本文を読みこなすのは難しいと思われる。
