Microbiology of wound infections among hospitalised patients following the 2005 Pakistan earthquake

2005年10月に壊滅的な地震がパキスタン北部を襲った後、パキスタンのシファ国際病院（Shifa International Hospital）に外傷のため入院した患者の微生物学的記録を後向きに解析した。地震の前後に創感染症を発症した入院患者の記録を比較検討した。年齢と入院期間を連続変数として解析し、平均値 ± SDを算出した。性別、損傷の種類、分離微生物、感受性、および増殖パターン（単数菌または複数菌）をカテゴリー変数として解析し、頻度を算出した。2群の相対リスクとその95％信頼区間を示した。年齢の平均値 ± SDは35.15 ± 11.11歳であり、患者数は女性のほうが多かった（58.4％）。最も頻度が高い損傷は骨折であった（55.9％）。最も検出頻度の高かった微生物はグラム陰性菌であり（89％）、複数菌感染（59.6％）および多剤耐性菌（61.5％）の割合が多かった。頻度が高い分離菌は緑膿菌（Pseudomonas aeruginosa）、エンテロバクター（Enterobacter）属、およびアシネトバクター（Acinetobacter）属であり、これらの多くは多剤耐性菌であった。

監訳者コメント：
外傷後創部感染症では、黄色ブドウ球菌やレンサ球菌も重要な起因菌であるが、この研究に示されているように環境由来グラム陰性桿菌を含む複数菌による場合が多い。
なお、別の報告ではイラク帰還兵に耐性アシネトバクター・バウマニー（A. baumannii）が多いとされており、中東の地域特性も注目される。
