Impact of organisation and management factors on infection control in hospitals: a scoping review

本スコーピング調査では、総合病院における感染制御に影響する組織・管理要因についてのエビデンスを探索した。文献検索により種々の研究、システマティックレビュー、および報告書が見いだされたが、質の高い直接的なエビデンスはわずかであった。研究の多くは観察研究であり、標準化された報告方法への準拠は概して不十分であった。病棟レベルおよび上層部の積極的なリーダーシップは、感染制御のための効果的な活動に必須であると考えられるが、監督下にある職員が多数である場合は、優れた臨床的リーダーシップによる効果が減少する。年長の臨床的リーダーには際だった存在感が求められ、その役割の範囲と責任を明確にすることも必要である。チームの安定性と意欲は、患者の転帰改善と関連する。また、訓練、評価、およびクリニカル・ガバナンスを支える組織構造は、効果的な実践と変革の成功をもたらすための重要な決定要因である。感染症発生率は、看護師の配置、病床利用率、および患者の入れ替わりなどに関する業務量と関連する。今回の調査で特定された組織の特性は、感染のリスク因子と考えるべきである。これらの因子は常に完全に排除・回避できるわけではないが、適切な評価を行うことによって、患者の利益を図るための活動が可能になると考えられる。

感染対策は人的要因が大きく影響するが、その質的評価は極めて難しい。現実としては、研究調査ではその実態が論文に反映されてしまっているのが実情であろう。
