抗感染症薬の処方を利用した連続的有病率調査による病院感染症の特定および監視★

著者らは、病院感染症有病率を推定するための「実用的な代替プロトコール(Pragmatic Proxy Protocol;PPP)」を開発した。このプロトコールは、現在薬局で行われている抗感染症薬の処方を利用した連続的な点有病率調査と、微生物学的マーカーおよび放射線診断マーカーの電子データを組み合わせたものである。本プロトコールを、標準的な英国病院感染学会・感染制御看護師協会プロトコール(Hospital Infection Society/Infection Control Nurses Association Protocol;HIP)と比較した。外科以外の患者のPPPの感度は1.00[95%信頼区間(CI)0.70~1.00]、特異度は0.97(95%CI 0.93~0.99)であった。このPPPの値から、外科以外の患者では、HIPによる病院感染症有病率推定値は42%低い可能性が示唆された。PPPに要する時間はHIPの3分の2(75時間対106時間)であり、費用は3分の1以上安価であった。PPPは電子報告システムの普及への対応が容易であり、「感染症治療のためのケア・バンドル(Anti-infective Care Bundles)」が開発されると、外科患者の病院感染症の検出感度は向上すると考えられる。PPPの使用によりルーチンの病院感染症サーベイランスの実施頻度が増加し、それによって感染症がもたらす総負荷が明らかになり、迅速かつ直接的なフィードバックおよび臨床医との連携を可能にする適時の介入戦略の効果を評価することが可能になると考えられる。
