市中獲得型メチシリン耐性黄色ブドウ球菌（Staphylococcus aureus）（CA-MRSA）は世界的に増加している。英国ではCA-MRSAの報告はあるが、その臨床的影響についての情報はほとんどない。本研究では、イーストヨークシャーで発生している3種類の流行株について報告し、その疫学、微生物学的特性、および臨床的影響について述べる。CA-MRSAは合併症を認めない皮膚・軟部組織感染症または無症候性の保菌に関連するものがほとんどであるが、重篤な感染症や死亡例も報告されている。2つの異なる家庭で市中獲得型の伝播が認められた。本稿では、CA-MRSAの既存の定義の限界に焦点を当てるとともに、CA-MRSAの新たな定義を行うため、および英国におけるその疫学と臨床的影響の研究を進めるための論点を提示する。

監訳者コメント：
CA-MRSAは、我が国では皮膚科領域で徐々に増加しているが、今のところ米国や英国ほどには伝播していないようである。これまでMRSAといえばHA-MRSAであり、各国のガイドラインやマニュアルはこれのみを対象としてきた。英国はCA-MRSAに対して既存の定義に限界を見いだし、来年度からは全入院患者に対して全例MRSAの積極的サーベイランス培養（ASC）を開始することになっている。我が国でもASCを簡便に行えるシステムの構築が2年以内に完了するように中期計画を立てておく必要がある。
