Incidence and risk factors for surgical site infection after simultaneous pancreas-kidney transplantation

膵・腎同時移植は、進行慢性腎不全を合併した1型糖尿病に対する最良の治療選択肢であるが、この処置を受けた患者の7％～50％に感染性合併症が生じる。2000年から2006年に著者らの施設でコホート内症例対照研究を実施し、膵・腎同時移植を受けた患者の手術部位感染（SSI）のリスク因子を評価した。評価を行った移植患者119例のうち55例（46.2％）がSSIを発症し、30日死亡率は11.8％であった。SSIから分離された微生物はグラム陰性菌が多かった。多変量ロジスティック回帰分析によりSSIと独立して関連する因子は、急性尿細管壊死、移植後瘻孔、および移植片拒絶反応であった。本研究から、この患者コホートではSSI発生率が高いこと、および外科手技と関係する因子がSSI発生と密接に関連することが示された。

監訳者コメント：
消化器系の移植手術は手術侵襲も大きく、免疫抑制を要するなどの理由で術後感染発生のリスクが高い。本研究でも手術症例の約半数がSSIを発生している。多変量解析で同定されたリスク因子は、いずれも介入困難なものであり、この種の手術のSSI防止対策の困難さを示している。
