Double-lumen central venous catheters impregnated with chlorhexidine and silver sulfadiazine to prevent catheter colonisation in the intensive care unit setting: a prospective randomised study

抗菌薬含浸カテーテルおよび消毒薬含浸カテーテルは、中心静脈カテーテル（CVC）への細菌定着を予防するために推奨される戦略である。クロルヘキシジン/スルファジアジン銀含浸カテーテルの集中治療室（ICU）の患者への使用に関するデータはほとんど報告されていない。クロルヘキシジン/スルファジアジン銀含浸CVC（第1群）と通常のCVC（第2群）の細菌定着率を比較する前向き無作為化研究を実施した。カテーテルへの細菌定着率を評価するため、無菌的に抜去したカテーテルの先端部4 cmをロールプレート法により培養した。全体で、カテーテルの挿入に成功した患者109例を試験に組み入れ、このうち51例を第1群に、58例を第2群に割り付けた。年齢、Sequential Organ Failure Assessment（SOFA）スコア、ICU入室時の診断、感染リスク、カテーテル挿入部位、およびカテーテル留置期間は、両群に統計学的有意差は認められなかった。細菌定着率は、第1群29.4％（カテーテル15本）、第2群では34.5％（カテーテル20本）であった（P = 0.50）。クロルヘキシジン/スルファジアジン銀含浸ダブルルーメンCVCには、ICU患者のカテーテルへの細菌定着率を減少させる効果はなかった。

監訳者コメント：
近年CV感染予防の方法として、抗菌薬や金属コーティングした素材によるカテーテルの使用やカテーテル内の抗菌薬ロック、カテーテル刺入部のパッドやドレッシング材に消毒薬や金属を使用した製品が多く供給されている。安全性は確保されていても、感染予防についての評価は製品群が様々であるため慎重な検討が必要である。
